Licensed Practical Nurse, School Based Health / Women & Children's SBH- Edward A. Reynolds West Side High School

Position Overview:

The Licensed Practical Nurse, as a member of the School-Based Health Center (SBHC) team will perform a vital role in engaging students in prevention education, screening, care and treatment. The LPN, along with all members of the team, will promote the SBHC’s services through various outreach activities as well as to individual students, family members and the school staff. The LPN also facilitates access to care by arranging and tracking appointments, referrals and follow-up visits, as directed by the SBHC practitioner.

Essential Functions:

  • Provide outreach to all eligible students and receive, interview and enroll new patients.
  • Screen patients by recording the chief complaint and taking and recording vital signs. Perform and record vision and audiometry screening.
  • Prepare patients for examination and assist in physical examinations and diagnostic procedures.
  • Administer medications and treatments as ordered, and carry out other specific orders issued by practitioner.
  • Perform phlebotomy and appropriate waived testing as requested by practitioner.
  • Provide patient education related to the visit and diagnosis and therapeutic interventions ordered or performed. Provide patients with treatment education materials, as needed.
  • Document in the electronic health record all education, counseling and health education provided to students. Immediately notify practitioner if there are any concerns about patient’s understanding of and/or concern about the care provided.
  • Provide first aid services as needed and, depending on the presenting condition, arrange for students to see the SBHC practitioner or, a practitioner at the Center. Respond and assist in emergency situations.
  • Practice accepted infection control measures in accordance with the OSHA Bloodborne Pathogens Standards and Center policies and procedures.
  • Keep and maintain all required logs including, but not limited to: refrigerator, freezer, first aid, emergency bag, and waived test logs.
  • Review immunization status of all students including reviewing the City-wide Immunization Registry (CIR) when necessary.
  • Make and record appointments and referrals for patients. Provide follow-up for missed appointments and abnormal labs. Escort students to appointments when necessary.
  • Process all Ryan Health, NYS Department of Health and NYC Department of Health forms. Maintain Medication Administration Forms for easy reference. Classify encounters as billable or non-billable.
  • Enter School-Based Health Center patient registrations, immunization, and demographic information into the School Health Online data system or another mandated database. Enter encounter data daily.
  • Reconcile the SBHC health records with the Department of Education records.
  • Maintain SBHC office and consumable medical supplies. Maintain medication and supplies inventory and Place orders for new supplies or medication as needed.
  • Promote the SBHC through routine group presentations to students. Maintain contact with school administrators, staff, teachers, and parents. Maintain contact with community agencies for the betterment of patients/students; make referrals as necessary or required to other facilities, Ryan, etc. work with parent groups associated with the school.
  • Call parents as needed to inform of ill or injured students and arrange student pick-up from school. Reinforce supportive care and practitioner’s recommendations to parents.
  • Attend and participate in staff and Departmental meetings, as required.
  • Attend and participate in outside meetings, health fairs, and other activities as required.
  • Provide services at the main Center as needed.
  • Immediately report any problems/unusual occurrences to supervisor.
  • Must demonstrate a professional, courteous, and respectful attitude in dealing with patients, immediate supervisor, and other employees.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served.
  • Perform other duties when assigned or when necessary.

  • This includes reporting any unusual occurrences of problems to the School Based Health Program Manager in a timely manner, attending all Center meetings as required and assigned, participating in performance improvement initiatives, and assisting in pre-visit planning activities.

Education, Experience and/or Skills Required:

  • High School Diploma and Graduation from an approved School of Practical Nursing.
  • Current New York State LPN license.
  • Effective communication and writing skills.
  • Proven track record of organization, attention to detail and ability to respond promptly to requests, anticipate organizational needs, and maintain efficient office operations.
  • Maintain current phlebotomy certification.
  • Maintain current CPR certification

Education, Experience and/or Skills Preferred:

  • Bilingual (English/Spanish) speaking desired.
  • Experience using an electronic health record.

Minimal Training Time:

90 actual workdays probationary period.

The work is performed primarily in an office setting. The noise level in the work environment may vary. Field visits and outreach are also performed at homes, hospitals or other services agencies.

When school is not in session or is closed, the School Based Health Center staff will be at the main Center or other sites as needed.
