MyCare Portal Registration Form

Before you proceed, if you are not a Ryan Health Patient please do not use this form.
You can register online as a new Ryan Health patient by clicking the corresponding button in the Patient Portal section of the Become a Patient page.

If you are a registered Ryan Health patient and have an active email address, you can request access to the MyCare Portal by submitting this form. Please fill out all fields and click submit. (At this time, patients who are 13-17 years of age are not able to use the MyCare Portal).
